If your name is not in the list, please submit your claim through any of the following means:
Filing of claims pursuant to the Insurance Commission’s Liquidation Order is until 16 September 2025 only. Claims filed after 16 September 2025 shall be BARRED from normal liquidation proceedings, BUT shall be reconsidered in CFPI’s dissolution and winding up proceedings.
CFPI shall regularly update this Claims Verification Portal.

  • All Planholders/Claimants are encouraged to UPDATE their CONTACT INFORMATION (i.e., cellphone and/or landline number[s]; e-mail; and physical address) on file with CFPI to ensure that the company may reach them whenever needed.
  • Checklist of Documentary Requirements to file a Pension Plan Benefits Claim:
    • Fully Accomplished Plan Benefit Claims Form
    • Photocopy (Xerox) of Pension Plan Agreement (Policy)
    • Photocopy of Certificate of Full Payment (If Available)
    • Photocopy of 2 Valid IDs of Planholder with 3 Specimen Signatures
      If the Planholder is Deceased:
    • Photocopy of Death Certificate of Planholder
    • Photocopy of 2 Valid IDs of Beneficiary with 3 Specimen Signatures
      Other Documents to Submit:
    • If Original Pension Plan Agreement is Lost or Damaged: Notarized Affidavit of Loss & Indemnity Agreement
    • If Claimant is a Representative of Planholder:
      • Notarized Special Power of Attorney
      • 2 Valid IDs of Representative
    • If Planholder is a Minor: Notarized Affidavit of Guardianship
    • If Claimant/Beneficiary is more than one: Notarized Waiver of Rights
