How to Register in CHS Member's Portal?

  • Visit our CHS website at www.caritashealthshield.com.ph and click Login.
  • Click Register Now to continue.
  • Fill out the Member’s Information System (MIS) Registration Form.
  • After completing the form, read the Privacy Notice, tick the box and click Submit.
  • Check your email and click the link for verification. You will then be led to the Member’s Information System (MIS).
  • Enter your Email Address and Password in the Member’s Information System (MIS) and click LOGIN.
  • Your account is now activated. Please click I am a Member to add your policy.
  • Enter your complete Policy Number . Click OK.
  • Kindly check if you have entered your Policy Number correctly. If correct, please click Confirm.
  • You have successfully added your Policy Number. Click Continue to proceed.

How to add policy?

  • Visit our CHS website at www.caritashealthshield.com.ph and click Login.
  • Enter your Email Address and Password in the Member’s Information System (MIS). If you are not yet registered, kindly click this link on How to Register.
  • Click LOGIN to start your session.
  • Welcome to Caritas Health Shield Member’s Information System! Click ADD POLICY.
  • Enter your complete Policy Number. Click OK.
  • Kindly check if you have entered your Policy Number correctly. If correct, please click Confirm.
  • You have successfully added your Policy Number. Click Continue to proceed.

How to apply for an electronic Letter of Authorization (e-LOA)?

  • Visit our CHS website at www.caritashealthshield.com.ph and click LOGIN.
  • Enter your Email Address and Password in the Member’s Information System (MIS). If you are not yet registered, kindly click this link on How to Register.
  • Click LOGIN to start your session.
  • Welcome to Caritas Health Shield Member’s Information System! Click APPLY e-LOA.
  • Enter the last 7 digits of your Policy Number. Click OK.
  • Kindly check if you have entered your Policy Number correctly. If correct, please click Confirm.
  • Select a Hospital or Clinic on the list.
  • Select the doctor’s Specialization on the list.
  • Select a Doctor on the list.
  • Review the Doctor’s Schedule. Click Submit.
    Disclaimer: The Doctor's Schedule is subject to change without prior notice. Please contact your provider of choice for confirmation of schedule or appointment prior to visit.
  • Click Yes to proceed or click No to cancel.
  • Check the generated e-LOA at the “Downloads” folder of your computer. Please take note that your e- LOA is password protected.
  • Save or print your e-LOA by clicking the or icon.

How to pay online using a Credit Card?

  • Visit our CHS website at www.caritashealthshield.com.ph and click LOGIN.
  • Enter your Email Address and Password in the Member’s Information System (MIS). If you are not yet registered, kindly click this link on How to Register.
  • Click LOGIN to start your session.
  • Welcome to Caritas Health Shield Member’s Information System! Click PAY ONLINE.
  • Enter the last 7 digits of your Policy Number. Click OK.
  • Kindly check if you have entered your Policy Number correctly. If correct, please click Continue.
  • Check the Policy Number, Full Name, and Amount. If correct, please click CONTINUE TO PAY.
  • If you are sure to proceed with the payment, please click Proceed.
  • Fill out the required fields in the Billing Information. Reminder: Please refrain from using special characters.
  • Fill out the required fields in the Payment Details. To proceed with the payment, click PAY.
  • Print your receipt as proof of payment. If you want to return to the website, please click Return to Website.
  • An email receipt (UB Receipt) for the payment made will also be sent to your email address.
  • For successful and error transactions, an Acknowledgement Receipt from CHS will be emailed to your email address.

How to update your contact information?

      Update your contact information in 3 easy steps:

  • Step 1: Log in to the Member’s Information System (MIS) portal. If you are not yet registered, kindly click How to Register .
  • Step 2: Click the Contact Information Update tab and fill out the fields.
  • Step 3: Click Submit. Expect to receive an SMS notification within the day. The change in your Policy Information will take effect the next day.
